It's important to note that certain requirements are likely to change, however, as of this very day, documents required are as follows:


  • Birth Certificate.
  • Local Government Certificate of Indigene/Attestation of Citizenship from Embassy.
  • If you have Non-Nigerian passport and hold dual nationality, upload data page of your Non-Nigeria Passport. 
  • Additional supporting document such as marriage certificate where applicable.
  • Police report for lost passport.
  • Sworn Affidavit for lost of Passport from High court.
